By creating a profile on and accessing The Loveland Therapy Portal (the “Portal”), you agree to the following representations, terms and conditions:

• You represent and agree that you are creating an account for yourself and not on behalf of another individual.
• You agree to provide accurate, current, and complete information when creating your account.
• You agree to promptly update all account information as necessary to ensure that it continues to be accurate, current, and complete.
• You agree that you will be solely responsible for any errors in information you enter in the Portal (for example, errors in bank account or mailing address information).
• You agree that you will be solely responsible for maintaining the confidentiality of your account credentials including your username and password.
• You agree to notify us immediately if you become aware of any unauthorized use of your account or any other breach of account security.

The Loveland Foundation reserves the right to suspend or terminate your account and your participation in the Program if you fail to abide by these Portal Terms and Conditions or any other requirements relating to the use of the Portal or participation in the Program, including the Terms and Conditions for Therapy Fund Program or the Terms and Conditions for Participating Therapists, as applicable.
